Medical Forms

The following Health and Medical forms must be completed for your child to attend St. Peter Catholic School:

Student Health & Medical Information
Physical Form  (Must be completed for all NEW students and for ALL Preschool through Kindergarten students)
Immunization Record (Must be completed for all NEW students and for ALL Preschool through Kindergarten students)

If medication is to be administered at school, please complete a Medical Administration in School or Child Care form to be signed by you and your physician.  A Health Care Plan must also accompany the Medical Administration in School or Child Care form:

General Health Care Plan (to include non-prescription medication such as cough drops and pain relievers)
Respiratory Health Care Plan (asthmatic students)
Health Care Plan for Severe Allergy (for use of  diphenhydramines and EpiPens)
Celiac Disease (Gluten Intolerance) 
Seizure Health Care
